ATHLETICS DROP FORM
Student Athletes should use this form to petition to drop a class.
If you are requesting to drop a class after the drop deadline (Friday of Week 7 in Fall/Winter/Spring or Friday of the week before the last week of your Summer Session, e.g., Week 5 of a 6 week session), you will need to provide verification from the instructor. We recommend you copy and paste the questions below, as petitions without these questions explicitly answered will not be reviewed. We will accept a PDF or JPG of an email from your instructor.
Verification is not required if you are requesting to drop a course before Friday of Week 7 of the current term or Friday before the last week of your Summer Session.
- If you are requesting to drop during the quarter or Summer Session and after the deadline (e.g., between Weeks 8-10, or during the final week of a Summer Session), the instructor must answer the following questions:

What is the course and the student’s name?
- Did the student complete all the coursework and/or take the final exam and/or submit the last paper/project/assignment?

- If you are requesting to drop after the last day of instruction (e.g., after Friday of Week 10 or the last day of the Summer Session), your instructor must answer the following questions:
- What is the course and the student’s name?
- Has the student ever attended class? If yes, when did the student first attend (approximate date or week)?
- Did the student take the midterm?
- Did the student complete all the coursework and/or take the final exam and/or submit the last paper/project/assignment?
- Date of the final exam, paper, or project.
- Based on the coursework completed (excluding the final exam/paper/project), what is the student’s grade at this time?
You will need to provide an explanation for your request. We recommend you draft this prior to initiating the petition and then copy it into the petition. Discuss the reasons you are requesting the drop and possible impact to eligibility. What extenuating circumstances or personal difficulties are you experiencing that is prompting this request?
You should submit any documentation that would support your explanation. You may upload multiple documents.
Please note that study list petitions cannot be processed if you have holds that prevent enrollment transactions. If you have a hold on your record, please return here to submit your petition after you have resolved the hold.
Because approval is not guaranteed, you should continue to attend classes until and unless your petition is approved.
